We are excited to announce RANKONE SPORT ONLINE FORMS. 

This is for High School and Middle School Marching Band, Cheerleading, Athletes and Athletic Trainers.

Please read all instructions before starting. There is a high rate of denying forms. Please take time and read instructions. If your child has listed a previous health condition, please list it on this medical history. Please do not put parent name where student name should be. Auto-fill may put parent name instead of Athlete's name. If possible turn off auto fill on your device. 

You will need to create an account with Rankone. Please DO NOT use continue as guest option.

You will need student id#, Grade, Date of Birth, valid email. Your Dr. Name and Phone #. (make sure you use Id# and not date of birth for the id#). 

Please use student's name not the parent. The questions are for the student not the parent. Please do not use auto fill, It may put the parent name instead of athlete. 

If you know you need a physical for 2020-2021 follow the steps below.

  1. Fill out the Student Information Form online in Rankone.
  2. Fill out the UIL Signature page online in Rankone.
  3. Print the medical history/physical form. (Copies are at the high school and middle school if you cannot print).
  4. Fill out the printed medical history form.
  5. Take medical history and physical forms to your doctor.
  6. Doctor will fill out the physical form part.
  7. Turn in printed completed and signed forms to your campus Athletic Trainer. You cannot upload forms in Rankone. You may drop them off at edge check in or the high School Front office. Please do not email forms. 

If you do NOT need a physical for 2020-2021 follow the steps below.

  1. Fill out Medical History form online in Rankone (only submit ONCE)
  2. Fill out Student information form online in Rankone
  3. Fill out UIL signature page form online in Rankone

Please make sure you fill out all questions and sign.

Once you have submitted all forms your child will still be marked as non-compliant.

They will remain non-compliant until the Athletic Trainer approves the forms.

The medical history will go blank after submitting.

Please do not fill Medical History out more than once, Unless it is denied, and you are asked to make some changes on the form.

Once the Athletic Trainer approves all forms, they will be marked as compliant. each form will be marked as approved

If you are a new student, you will not be in the system. You will need to email Coach Carrell with full name, DOB, address, grade and school ID#. Coach Carrell will put information in system and then you will be able to fill out online forms. 

If you notice this: ERROR-Student information does not match any existing student: try some of the suggestions listed below:

fill out all information. first name, last name, and DOB. If you still get the error contact Coach Carrell. tcarrell@ssisd.net

Recommend using Google browser. some issues when using safari browser. 

A few reasons for Medical History being denied.

          Not listing previous concussion

          Not listing previous family heart history

          Not listing previous injuries and/or surgeries

          Wrong birth-date, wrong name listed

          Putting parent name instead of student name

          Not answering all questions, questions are about the athlete

If you are not sure why your medical history is denied, please email Coach Carrell. Please do not resubmit with the same answers. Only submit medical history once, unless you are asked to make changes.
