SCHOOL PHYSICALS WILL BE SUNDAY APRIL 28TH, FROM 2-3PM AT THE HIGH SCHOOL, COST IS $15.00
This is for High School and Middle School. This includes Marching Band, Cheerleading, and Athletes.
We have some forms that will be read online. They will not
need to be printed and turned in. However, you will sign the SSISD Information form acknowledging that you have read all the forms
online. This must be completed and signed before participation.
There will be a few forms that will be required to be completed, printed, and returned.
Athletes will return forms to the campus Athletic Trainer.
Band students will return forms to their Director.

Information Form must be completed every year.
This form is 2 pages. The first page is The Medical History Form. This
page must be completed every year. Please complete this form, sign,
print, and return to the campus Athletic Trainer/Director. The second page is the
Physical form. This page will be completed by a Physician and then returned to the campus Athletic Trainer/Director.
Please read the requirements for the physical form.
All students entering the 7th, 9th, or 11th grade will need a new physical.
All students entering the 8th, 10th, or 12th grades may need a new physical as well. explanation below.
On the Medical History form any Yes answer to questions 1, 2, 3, 4, 5,
or 6 requires further medical evaluation which may include a physical
examination. Written clearance from a physician, physician assistant,
chiropractor, or nurse practitioner is required before any participation
in UIL practices, games, contest, or matches.
If you have ever had a concussion you will be required to get a new physical every year.
If you have ever been diagnosed with a heart murmur you will be required to get a new physical every year.
All Marching Band students will be required to get a physical this year due to the new requirements.
